The oral drugs currently available in Australia for the treatment of cutaneous fungal infections include griseofulvin, ketoconazole, fluconazole, itraconazole and terbinafine.

IntroductionThe cutaneous mycoses are superficial fungal infections of the skin, hair or nails. Essentially no living tissue is invaded however, a variety of pathological changes occur in the host because of the presence of the fungus and/or its metabolic products. The principal aetiological organisms are:

  • dermatophytic moulds belonging to the genera Microsporum, Trichophyton and Epidermophyton which cause ringworm or tinea of the scalp, glabrous skin and nails
  • Malassezia furfur, a lipophilic yeast responsible for pityriasis versicolor, follicular pityriasis, seborrhoeic dermatitis and pityriasis capitis
  • Candida albicans and related species, causing candidiasis of skin, mucous membranes and nails

The usual approach to the management of cutaneous infections in immuno competent patients is to treat with topical agents. However, nail and hair infections, widespread dermatophytosis and chronic non-responsive yeast infections are best treated with oral antifungal drugs.

Dose Regime For Oral Fluconazole

For vulvovaginal candidiasis, a single oral dose of fluconazole 150 mg is usually effective. It can be repeated.

For dermatophyte infections and pityriasis versicolor, either 50 mg daily or 150 mg once weekly is taken for two to six weeks.

Once-weekly fluconazole is often used off-label to treat toenail fungal infections .

Larger doses are required for systemic infections.

Fluconazole is not normally used in children but doses of 5 mg/kg/day have been safely prescribed for a serious infection.

Drugs That Should Not Be Used With Fluconazole

There are certain drugs that you should not use with fluconazole. When used with fluconazole, these drugs can cause dangerous effects in your body. Examples of these drugs include:

  • Terfenadine. When used with fluconazole at doses of 400 mg or higher, this drug can cause a life-threatening heart rhythm condition called torsades de pointes.
  • Pimozide, clarithromycin, erythromycin, ranolazine, lomitapide, donepezil, voriconazole, and quinidine. When used with fluconazole, these drugs can cause a life-threatening heart rhythm condition called torsades de pointes.

What Other Drugs Will Affect Fluconazole

Sometimes it is not safe to use certain medications at the same time. Some drugs can affect your blood levels of other drugs you take, which may increase side effects or make the medications less effective.

Fluconazole can cause a serious heart problem. Your risk may be higher if you also use certain other medicines for infections, asthma, heart problems, high blood pressure, depression, mental illness, cancer, malaria, or HIV.

Many drugs can affect fluconazole, and some drugs should not be used at the same time. Tell your doctor about all your current medicines and any medicine you start or stop using. General Advice If You Are Diagnosed With Scalp Ringworm

If you are confirmed or your child is confirmed as having scalp ringworm then:

  • Where possible, you should throw out or disinfect objects that may be contaminated with fungal spores – for example, hats, combs, pillows, blankets and scissors. This aims to prevent re-infection after treatment and to prevent passing on the infection to others.
  • You should not share towels. Wash used towels frequently.
  • It is not necessary to keep children off school. However, to ensure that the infection is not transmitted to others, they should carefully follow the recommended treatment.
  • Monitor the scalps of other children in the household. If there are any signs of scaling or hair loss then seek medical advice.
  • If a pet is suspected of being the source of the fungal infection then seek advice from a vet for treatment.

Pregnant women: Research in humans has shown adverse effects to the fetus when the mother takes this drug in doses of 150 mg or higher. In lower doses, research in animals has shown adverse effects. This drug should only be used during pregnancy in serious cases where its needed to treat a dangerous infection in the mother. And it should only be used if the potential risk to the fetus is acceptable given the drugs potential benefit.

Scalp Yeast Infection And Hair Loss

If your scalp yeast infection lasts for a long time, you may accumulate a lot of flakes and dead skin. Frequently scratching or applying drying chemicals to the affected area can also damage hair follicles.

Do not take any other medicines to treat diarrhoea without speaking to a pharmacist or doctor.
  • feeling or being sick stick to simple meals and do not eat rich or spicy food. If you are being sick, try small, frequent sips of water to avoid dehydration. It might help to take your fluconazole after a meal or snack.
  • rash it may help to take an antihistamine, which you can buy from a pharmacy. Check with the pharmacist to see what type is right for you. If the rash gets worse, or does not get better after a week, speak to your doctor.

Drug Interactions With Fluconazole

Unfortunately, fluconazole can interact with other medications.

  • Fluconazole should not be taken with the HMG Co-A reductase inhibitor simvastatin, and the dose of atorvastatin should be reduced. Pravastatin and rosuvastatin are preferable. Toxicity results in muscle pain and weakness, which may be serious.

Over The Counter Supplements Vs Prescription Medications For Candida

It may be tempting to think or believe that prescription medications are always more powerful than over the counter supplements.

And while this may make sense intuitively, it certainly isn’t always the case.

Some studies, such as this, have shown that herbal antibiotics/antifungals can be just as potent than conventional prescription medications.

Having said that, prescription medications certainly have their place, especially when treating gut-related issues such as SIFO and SIBO.

Antifungal Creams And Shampoos

Antifungal creams or shampoos do not clear the infection fully. This is because fungi get into the hair shafts where creams and shampoos cannot reach. Therefore, they cannot cure the condition. They do, however, clear fungi and fungal spores from the hairs and surface of the scalp.

An antifungal shampoo or cream may be advised twice-weekly for a couple of weeks in addition to antifungal medication – for example, selenium shampoo, ketoconazole shampoo or terbinafine cream. This is to clear any surface fungi quickly and make it much less likely for the fungus to spread to other children.

Who Can And Cannot Take Fluconazole

Most adults and children can take fluconazole. It can also be prescribed for babies.

Fluconazole is not suitable for everyone. Tell a pharmacist or your doctor before taking it if you have:

  • had an allergic reaction to fluconazole or any other medicines in the past
  • heart disease, including heart rhythm problems
  • kidney or liver problems
  • a rare, inherited blood disorder called acute porphyria
  • had a blood test that showed you have abnormal levels of potassium, calcium or magnesium

What If I Forget To Take A Dose

If you forget a dose, take it as soon as you remember, unless it’s nearly time for your next dose. In this case, just skip the missed dose and take your next one as normal.

Do not take a double dose to make up for a missed dose.

If you forget doses often, it may help to set an alarm to remind you. You could also ask your pharmacist for advice on other ways to remember your medicines.

Can Fluconazole Cause Problems

Along with their useful effects, most medicines can cause unwanted side-effects although not everyone experiences them. The unwanted effects often improve as your body adjusts to the new medicine, but speak with your doctor or pharmacist if any of the following continue or become troublesome.

Candida Infection Of The Skin

Candida infections of the skin respond well to a range of antifungals available in cream, powder, or solution formulations . Useful antifungals for candidosis are azole drugs . Other antifungal agents, not suitable for dermatophytosis, are the topical forms of the polyene antifungal drugs, such as nystatin, amphotericin B, and natamycin . Generally, superficial infections due to Candida of the skin and mucous membranes respond well to these treatments. However, most clinical trials have focused on oropharyngeal or vaginal infections, or cutaneous involvement in chronic mucocutaneous candidiasis .

Flexural candidosis. This refers to Candida infection in flexures such as the groins or inframammary fold, sometimes called Candida intertrigo. This requires topical therapy given for 2 weeks , but treatment may be continued for longer periods. Drying the infected site is also important in many cases where the moist skin surface can cause much discomfort.
