What Is The Anatomy Of The Nail

Looks Like Nail Fungus: Treating and Trimming Fungal Nails

To have a better understanding of how fungal nail infection affects the nail, a general knowledge of the anatomy of the nail is helpful . The nail, or nail unit, consists of the following parts:

  • The nail matrix is where nail cells multiply and keratinize before being incorporated into the fingernail or toenail. Most of the matrix is not visible. The matrix starts under the skin 5 mm below the nail fold and covers the area called the lunula, or half moon .
  • The cuticle is a fold of modified skin where the finger or toe meets the nail. The cuticle protects the matrix from infection.
  • The nail plate is the nail itself.
  • The nail bed is the soft tissue underneath the nail, anchoring the nail plate. The nail plate protects the nail bed.

What If I Decided Not To Have Any Form Of Treatment

Onychomycosis is not life threatening and many have lived with the condition for years however, it can make someone more susceptible to secondary bacterial infection, particularly of the lower legs in those with poor circulation. Likewise, it may impose psychological stress because of the social stigma it carries and impact on daily life. Onychomycosis may also adversely affect ones occupation as it may affect the ability to perform certain tasks.

In a quality of life assessment of people affected by onychomycosis, the constant reoccurrence as well as the number of nails affected was shown to negatively affect quality of life. Moreover, 64 74% of those surveyed, admitted to embarrassment and lower self-image/self-esteem.

Treatment Of Nail Fungal Infection At Different Stages

Treatment of onychomycosis at the first stage is crucial as the infection hasnt exacerbated yet and therefore is easier to heal. Topical treatments, such as anti-fungal creams and gels or medicated nail polish are the most commonly used at this stage. However, it is ideal to use a topically-applied medication that not only can kill the fungus, but can penetrate the nail as well.

Most topical treatments dont work because they cant penetrate the nail to get to the fungi. It is therefore recommended for sufferers to use a topical medication that can penetrate the nail and kill the fungus. A solution with 10% undecylenic acid, which can effectively get rid of the infection, and a penetrating agent , is recommended.

For moderate nail fungus infection, such topical solutions can still work. However, many sufferers use that in conjunction with oral medicines. Many anti-fungal pills in the market can treat fungal nail infection, though it is still suggested to use such medicines along with topical solutions .

Unfortunately, oral medicines are also more likely to cause side effects to patients. They are also not recommended for the elderly, pregnant women, and those who have other medical conditions or ailments.

But again, fungi can come back, even without the nail itself. This is why its recommended for patients to use topical anti-fungal ointment to the nail bed and surrounding skin while waiting for the nail to grow back. Otherwise, the fungi will just return.

What Is Toenail Fungus

Toenail fungus is an infection that gets in through cracks in your nail or cuts in your skin. It can make your toenail change color or get thicker. It can also hurt. Because toes are often warm and damp, fungus grows well there. Different kinds of fungi and sometimes yeast affect different parts of the nail. Left untreated, an infection could spread to other toenails, skin, or even your fingernails.

What Other Conditions Can Be Mistaken For Fungal Nails

Images of toenail fungus

Here are some other conditions you may have instead of fungal nails:

  • Lines and ridges: These are common and may be considered normal. They may worsen during pregnancy. A large groove down the center of the nail can be caused by nail biting. Some people may develop these changes following chemotherapy.
  • Senile nails: As you age, the nails become brittle and develop ridges and separation of the nail layers at the end of the nail. To avoid this, try to clean solutions and dont soak the nails in water.
  • Whitish or yellowish nails can occur due to onycholysis. This means separation of the nail from the nail bed. The color you see is air beneath the nail. The treatment is to trim the nail short, dont clean under it, polish if you want to hide the color, and wait two to three months. Persistent onycholysis can make the nails susceptible to fungal infection.
  • Red or black nails due to a hematoma, or blood under the nail, usually occur from trauma . The discolored area will grow out with the nail and be trimmed off as you trim your nails. If you have a black spot under your nail that was not caused by trauma, you may want to see a dermatologist or a podiatrist if it involves a toenail to make sure it is not melanoma . A simple biopsy can rule out malignancy .
  • Trichophyton

    Treating Fungal Nail Infection

Treatment may not be necessary in mild cases of fungal nail infection. For more severe or troublesome cases, antifungal medication may be recommended.

A fungal nail infection is unlikely to get better without treatment, but if youre not bothered by it you might decide its not worth treating because treatment can take a long time, may cause side effects, and isnt always effective.

Whether or not you decide to have treatment, you should still follow the self-help advice below to help stop the condition getting worse or spreading to others.

When Are The Different Treatments Considered

Most doctors recommend treating nail fungus with nail polish or cream if

    • not much more than half of the nail is affected by the fungus,
    • the base of the nail is not infected, and
    • only some nails are affected.

Topical treatment is also usually recommended for children. One reason for this is that most oral medications arent suitable for children. Another reason is that children have thinner nails that grow more quickly, so its assumed that treatment with nail polish or creams is more likely to work in children than in adults. White superficial onychomycosis is also often treated with a nail polish or cream.

If several nails are infected by the fungus, or if the infection has spread out more on the affected nails, its usually necessary to take oral medication. And if the infection started at the base of the nail, its highly likely that only tablets will help.

How Can Someone Get An Infection

Onychomycosis is contagious. It is transmitted by direct contact or contact with contaminated materials such as shoes, socks, or walking barefoot over surfaces with a high density of fungal or yeast spores.

Although it is not a hereditary condition, not everyone is equally susceptible. There are families with genes that make them more prone to fungal nail infections. They are the ones who usually get onychomycosis at a younger age and those with repeated infections. Likewise, there are genes that have been shown to protect against acquiring the infection.

Immune dysfunction, particularly a defect in cell-mediated immunity also makes one more predisposed to onychomycosis. This group includes those on prolonged steroid therapy, those that are malnourished, or those with chronic illnesses such as diabetes.

Age and sex also play a role. Fungal nail infection is more common in males and one becomes more susceptible with increasing age. Children are less likely to have onychomycosis compared to adults and the incidence of onychomycosis may be as high as 60% in the elderly.

Lifestyle and certain activities may also be contributing factors. People who frequent public swimming pools, gyms, or locker rooms are more exposed to onychomycosis. Also people whose feet sweat or are frequently injured, are at increased risk of getting the infection.

    • Surgery. If other treatments donât work, the doctor may need to remove your nail entirely and let a healthy one grow back in its place. The new nail could also get infected.
    • Laser or photodynamic therapy. Doctors are studying newer treatments that use special light to try to kill the fungus.

    • Use the treatment exactly as prescribed

    • Apply the medicine for as long as prescribed

    • Keep all follow-up appointments with your dermatologist

Differences In Appearance Between Blood Under A Toenail And A Melanoma: Under More Scrutiny

Association with trauma. The trauma that leads to blood under a toenail isnt always blunt and painful at the time of occurrence.

It could be as seemingly innocuous as jogging downhill in footwear that isnt roomy enough. The patient might not even recall the trauma.

Though subungual hematoma often affects both big toes at the same time, it CAN occur in only one toe.

If your right foot is a little bigger than the left, the shoe on your right foot will have a tighter fit that may result in a hematoma, while the shoe on the left wont.

More than one toenail. If the appearance of the dark pigment in a big toe coincides with a similar-looking pigment in another toe either on that foot or the other then this points to a benign cause.

Blood grows out. If you view a hematoma every several days, youll notice that the space between the portion closest to the cuticle and the cuticle is getting bigger, or showing more nail.

As the nail is pushed out, the blood underneath goes with it. So if you noticed the pigment when it was first visibly connected to the cuticle area, in a few weeks it will be closer to the center of the nail, then after several more weeks itll be near the top of the nail. It can take several months to finally grow out.

Eventually the nail will be long enough such that when you trim it, youll also be trimming out the old dried blood.

A melanoma will stay firmly adhered to its place it will not move upward as the nail grows.

    How Is Onychomycosis Diagnosed

Physicians can identify fungal nail infection by performing an examination most of the time. There are times when it becomes difficult to tell onychomycosis apart from other conditions that cause nail abnormalities such as psoriasis and trauma. A sample may be taken from the debris beneath the nail and examined microscopically or cultured for possible fungal growth. This laboratory procedure is important not only to confirm the diagnosis but also to identify the type of fungus and help determine the best treatment to be given.

Your physician may request certain blood tests to check your liver function to ensure that it is safe for you to take the pills.

    How Fungi Infect The Nail

Onychomycosis, or toenail fungal infection, is an invasion by a microscopic organism that thrives in warm, damp environments. Fungal spores are in the air, and they will grow if they land on a receptive surface like your toenail. They feed off the nail tissues, burrowing into the skin under the nail. Over time the nail thickens and may lift off the nail bed as fungal debris accumulates. Once your nail is raised off the nail bed, it wont reattach, and a new nail wont grow from that part of the nail bed. However, your nail will continue to grow from the root at the base.
