Does Jublia Work To Treat Toenail Fungus
Jublia, recently approved by the FDA to cure toenail fungus has been touted as the latest and greatest treatment for this stubborn condition, but does it really work? Jublia and another recently approved medication, Kerydin, are topical versions of a new class of antifungals called oxaboroles. They are believed to be better at penetrating the thick nail plate to reach the fungus growing beneath it. Current over-the-counter and prescription antifungals are not effective in penetrating the nail bed. Oral antifungals have been used for years but have potentially dangerous side effects and can interact with other medications.
Although a stronger and more localized treatment sounds promising, both Jublia and Kerydin have mediocre results at best. First of all, these medications need to be applied daily for 48 weeks, and can have potential side effects, such as ingrown toenails, redness, itching, swelling, burning or stinging, blisters and pain. Secondly the cost is prohibitive, a small bottle going for $560 dollars not covered by most insurance. Thirdly, during the entire year of using Jublia, you are prohibited from getting a pedicure or painting your toenails. Most importantly, however is the low efficacy rate. In clinical trials daily use of Jublia for 48 weeks resulted in complete cure of nail fungus in 15-18% of patients. Daily use of Kerydin resulted in cure for 6-9% or study subjects.
Jublia Vs Ciclopirox: Ease Of Use
One issue a lot of patients have with ciclopirox is that because it is oil-based, it goes on a bit sticky and takes a few minutes to dry. It will also build up and look yellowed. Jublia on the other hand has an alcohol base and goes on much cleaner and drys quicker. It also doesnt build up and look yellowed.
To summarize, the best topical medications work between 10% 20% of the time to make the nail look better. The new ones work a little better than the Ciclopirox, but are much more expensive, although discount coupons are currently available and they may be covered by insurance. Of the three, Jublia probably works the best and Ciclopirox is the most cost effective. While Jublia might work a bit better, we dont feel that it offers enough of an improvement to justify the cost so we usually prescribe Ciclopirox. If a patient wants to try it, however, we will provide the prescription. We dont generally prescribe Kerydin as it is just as expenive as Jublia and from the studies we have seen not quite as effective.

What Is Toenail Fungus
Toenail fungus is also referred to as onychomycosis . Fungal infections can originate from a variety of molds, yeasts, and fungi that can grow in the cracks of the nail or skin surrounding the toenail. Athleteâs foot, often contracted from locker room floors, is a type of fungus that can lead to an infected toenail or skin infection. The most typical fungus that grows on the feet and leads to needing treatment is called dermatophyte.
Fungal Nail Symptoms And Risks
Jublia uses entail treating fungal nail symptoms, which may include:
- Thickened nails
This can progress to athlete’s foot if it travels into the skin. The official term for athlete’s foot is ringworm, and it is a fungus that presents with a circular rash. How do fungal infections happen?
Many yeasts and mold live in your home, gyms, and other buildings. When your skin or toenails crack, it is more susceptible to infections. Individuals with diabetes, poor immune systems, or blood circulation issues are the most at risk.

How Is Toenail Fungus Diagnosed
Nearly 3 million people in the United States are diagnosed with toenail fungus each year. Men are up to three times more likely to develop toenail fungus than women. You are also at a higher risk for a toenail fungus infection if you smoke or have diabetes. Also, individuals with compromised immune systems may experience toenail fungus that can turn into an infection.
When feet remain moist or damp, the fungus can grow, causing discoloration and thickening of toenails. Infection can cause discomfort and pain. The spread of fungus and infection can go from toe to toe, hand, fingernails, or skin on other parts of the body.
The toenail may become warped, cracked, or loosen itself away from the nail bed. Often the nail appears yellowish and may have white spots. Your doctor will examine your feet and potentially take a small sample of skin or nail to be sent off to a laboratory for testing. Your doctor may refer you to a dermatologist, podiatrist, or other healthcare professional if your condition is unusual or needs special care. Other foot conditions such as psoriasis can look similar to a fungal infection. Lab reports can indicate if the problem is fungal, helping the doctor prescribe the best medication for effective treatment.

Cliclopirox Vs Kerydin Vs Jublia Which Topical Is Best
Our preferred topical is 8% ciclopirox lacquer for daily use for 12 months after laser or oral medication treatment then once or twice a week for maintenance. The advantages of ciclopirox are that its one of the most effective topical medications and inexpensive and may be covered by insurance if prescribed. The downside is that its a hassle to apply, must be removed weekly and you cant use nail polish or other nail products while in treatment.
Kerydin and Jublia and are newer topical medications for nail fungus and, in fact, work a little better than ciclopirox according to most studies. In addition, they are easier and cleaner to apply. Ciclopirox goes on kind of sticky as it is oil based. Kerydin and Jublia are alcohol based and so are much cleaner to put on. Unfortunately they are much, much more expensive . There are some coupons available for the Kerydin and Jublia that can lower the cost considerably. If you are paying for it out of pocket without a discount, however, we dont think that the the minor improvement in outcomes is worth the cost for most patients, but we do occasionally prescribe them to patients who want to avoid the mess of ciclopirox.

How Do I Apply For The Jublia Patient Assistance Program
Our process makes it as easy as possible to apply for the Jublia patient assistance program. Begin by completing the enrollment application form on our website. Tell us about any medications that you are taking, including Jublia, your healthcare provider, your insurance plan, and your household income. This information is required by the pharmaceutical manufacturers who will supply the medication, so it is important to make sure this information is accurate and up to date.
We will review the information that you have provided to determine whether you are eligible for patient assistance. If we can pre-qualify you for assistance, NiceRx will handle the full enrollment process on your behalf. If you are approved by the pharmaceutical company, you will typically start receiving your medication in 6 8 weeks.
Evaluation And Diagnosis Of Fungal Nails
We will first examine your feet and nails and take a history. This can help determine if you have a fungal infection or nail dystrophy. After doing a history and examination to help determine the cause of your nail changes, we may get a lab test. This involves taking a small sample of the nail and having it examined in the lab under the microscope.
The test is called a Periodic Acid-Shiff test which involves a stain that turns any fungi red. This will also help identify the exact type of fungi present so that we can offer you the most effective treatment. This is often the only way to determine the difference between fungus and dystrophy as they can look exactly the same.
This is a critical step as we see many patients who have spent hundreds of dollars on laser treatment for nail fungus or have taken oral anti-fungal medications only to find that there is no fungus present and that there was no reason to even consider these treatments. Sometimes visual examination of the nail is adequate to diagnose toenail fungus, but many times it is not.
During your first visit with us we will provide you with a plan of action for treating the dystrophic or fungal nails. We may be able to provide you with recommendations on the first visit but we often have to wait for lab tests to be completed. Thus, we cannot guarantee that any specific treatment will be performed during your first visit.

The Generic Version Of Jublia Is Not Yet Available
Generic Jublia is not yet available in the United States. The earliest possible date a generic version will become available is sometime in 2030, which is when the patents are expected to expire.
Generic drugs are usually less expensive than brand-name drugs. When a generic version of efinaconazole becomes available, there will likely be more competition and the price of efinaconazole may go down.

Toenail fungus isnt a pressing health problem. Yet a fungal infection can ruin the appearance of your nails and cause pain as it lifts the nail away from the nail bed. And fungal infections are notoriously difficult to get rid of. Most often, you should consider fungal toenail a condition to be managed rather than cured.
