How Do Dermatologists Diagnose Nail Fungus

To find out if a patient has nail fungus, a dermatologist examines your nails and nearby skin. Its important to check the skin because the fungus can spread. You may already have a skin infection caused by fungus like athletes foot.

When it comes to off-the-shelf toenail fungus products, theres probably no harm in using these medications, says Dr. Lipner. But the efficacy is not very good. So youre probably wasting your time, and youre probably wasting your money.

Same with the home remedy of using Vicks VapoRub. It works okay on a not-so-common type of foot fungus, but for the one most people have, it doesnt work well, she says. I think people believe that OTC products are safer in some way, but the reality is that they havent been approved by the FDAthey havent gone through extensive testing on thousands of patients. Why would you use products that havent been very well studied?

How Effective Are Topical Treatments

So far, only a few studies have looked into topical nail fungus treatments with nail polishes or creams. Because these studies had weaknesses, the results should be interpreted with caution. Amorolfine has not yet been well studied. Ciclopirox polish and treatment sets with urea and bifonazole cream were tested in a few studies.

Research on the effectiveness of nail polish containing ciclopirox showed that, after one year:

• About 10 out of 100 people who did not use ciclopirox no longer had a detectable fungal nail infection.
• About 32 out of 100 people who used ciclopirox no longer had a detectable fungal nail infection.

In other words, treatment with ciclopirox got rid of the fungal infection in about 22 out of 100 people. But even if the fungus had gone away, the cosmetic result wasnt always satisfying. The nails only looked healthy after treatment in 7 out of 100 people.

Treatment with sets containing urea and bifonazole cream was tested in one study. It was compared with a treatment in which only urea cream was applied and the nail was removed, but without applying bifonazole cream afterwards. Three months after treatment was completed, it was found that:

• No fungus was visible or detectable in about 41 out of 100 people who only used urea cream.
• No fungus was visible or detectable in about 51 out of 100 people who used both urea and bifonazole cream.

Toenail Fungus Treatment Options

Treatment of onychomycosis will vary, depending on the severity of the infection and the type of fungus that initially triggered the infection. Topical treatments like antifungal creams and medicated nail lacquers are among the most effective treatment options for toenail fungus.

Laser treatment or the surgical removal of the toenail may be necessary in more severe cases of toenail fungus. Over-the-counter products and home remedies are also methods used alone or in conjunction with prescription drugs to treat nail fungus and prevent reinfection.

What Are Common Side Effects Of Toenail Fungus Medication

Topical medications often prescribed to treat toenail fungus can potentially irritate the skin. Itchy, peeling, red flesh and blisters may also occur with many topicals. Consult your doctor if your medication is causing an unwanted skin reaction.

Liver damage is one of the most concerning side effects of some toenail fungus medications. Due to the nature of onychomycosis, many of the medications need to be taken for long periods of time to completely resolve nail fungus. Gastrointestinal issues, dizziness, and trouble breathing are also listed as potential side effects. In addition, if the individual treating the toenail fungus suffers from heart conditions, the medication could interfere with heart health.

What Causes Tinea Unguium

A type of mold called a dermatophyte causes tinea unguium. Dermatophytes are fungal microorganisms . They feed off of keratin, a protein found in your fingernails and toenails. Keratin makes nails hard.

Dermatophytes are the cause behind 90% of toenail fungal infections. But other types of fungi can infect your toenails as well.

What Is The Treatment For Fungal Nails

Oral antifungal therapy has a high cure rate, depending on the medication. It can take nine to 12 months to see if it has worked or not, because that is how long it takes for the nail to grow out. Even when therapy works, the fungus may come back. Currently, an oral antifungal therapy is considered the best treatment for toenail fungus because of higher cure rates and shorter treatment duration compared to topical therapy.

Prescription oral medications that are effective against nail fungus include the following:

There are several innovative treatments that are still being tested:

• Laser therapy or uses application of light-activated agents onto the nail followed by shining light of a proper wavelength on the nail.
• Use of electrical current to help absorption of topical antifungal medications into the nail: This is also called iontophoresis.
• Use of a special nail lacquer that changes the micro-climate of the nail to make it inhospitable for the fungus to grow: If this works, it may be an inexpensive way to treat this problem in the future.

One way to definitively get rid of toenail fungus is by surgery. Surgical treatment of onychomycosis involves nail removal. However, this often only provides temporary relief, and recurrence is common unless additional antifungal medication is simultaneously used. However, surgical removal may be warranted when the affected nail is associated with other factors such as trauma and or infection.

What Are The Causes

You can pick up a fungal infection such as toenail fungus while walking barefoot at a swimming pool, in a gym locker room or shower, or even when having a pedicure under less-than-sanitary conditions. The fungus thrives in warm, dark, moist environments, like showers or sweaty shoes. Susceptibility may be heightened among those who have diabetes, an immune deficiency condition, a history of athletes foot, or who are prone to excessive perspiration. Foot and toenail fungus usually will travel through an opening in the skin or separation in the nail bed, and if continually exposed to warm and moist environments, will grow in abundance. Infection with nail fungus seems to occur more in toes than fingers because toenails usually are confined to the dark, warm, and moist environments where fungus thrives, as opposed to fingers, which are out in light and open air. Diminished blood circulation to the toes as compared with the fingers can also make it harder for the bodys immune system to detect and eliminate the infection.

Its estimated that 10% of the population deals with stubborn toenail fungus.1

Sometimes itâs possible to treat your toenail fungus at home. You can try:

• Over the counter antifungal creams or ointments. File or cut off any white markings on your nails. Soak your nails in water and dry off before you put on the medicated cream.
• Special nail care. If you keep your nails trimmed, it can reduce the pressure and pain you feel. You can also thin your nail bed by filing it down. This can help antifungal creams get to the problem easier.

What Are The Signs Of Nail Fungus

If you get nail fungus, youre likely to see one or more of the following changes to your nails:

• Part of a nail turns white, yellow, brown, or another color. At first, you may just see a spot of discoloration at the tip of your nail. Without treatment, this discoloration may spread, covering more of the nail.

• Debris builds up under the nail.

• A nail begins to lift up, so its no longer firmly attached to the finger or toe.

What Side Effects And Drug

The possible side effects of itraconazole include headaches, dizziness, stomach and bowel problems, and rashes. Itraconazole can also interact with a number of other drugs. These include cholesterol-reducing and blood-sugar-lowering medications, as well as certain sleeping pills. It is therefore important to let your doctor know about any medication you take. Are Oral Medications For Nail Fungus Toxic

The newer drugs are unlikely to cause any liver problems in patients without known liver disease. Blood tests are not needed for once-weekly treatment with fluconazole however, people taking longer courses often have their liver function tested before starting the medicine and then retested during the course of treatment. It is important to notify the doctor of all side effects while on the medication. You should tell your doctor of all current medications to prevent potential serious drug interactions.
