Impact Of Fungal Diseases In The United States
Fungal diseases can cause serious illness and death. Scientists are still learning about how many people in the United States are affected.
The impact, or burden, of these diseases is difficult to estimate because:1

Assessment Of Therapeutic Endpoints
Clinical and microbiological endpoints were systematically assessed. Patients and physicians evaluation of the therapeutic response was collected from the patients records. In addition, an independent dermatologist evaluated all photographs that had been taken before every session and at the final visit.
The primary endpoint, clinical improvement, was defined as a significant clinical improvement evaluated by the patient himself and assessment of the photographic documentation by an independent physician. Clinical cure was defined as 100% clear nail with no subjectable clinical signs of fungal nail infection via assessment of the photographic documentation by the independent physician. Furthermore, at the end of the last laser treatment, a final fungal culture and microscopic assessment of mechanically removed nail material were performed. Secondary endpoints were accordingly defined as the clearance of fungal pathogens in microscopic examination and in fungal culture after up to 4 weeks of incubation. Direct fluorescence microscopy assay was used for microscopic examination. Therefore, Blankophor was used as a fast fluorescent whitener and direct detection of fungal elements was determined using a Zeiss Axioplan fluorescence microscope.
Fungal growth and spore production in culture medium were performed on Sabouraud agar for up to 3 weeks at room temperature. Mycological cure was defined as both negative microscopy and negative culture.

She was concerned it might be a fungal infection and wanted to treat the condition before it worsened. I obtained an H/P, a specimen for culture and discussed treatment options depending on culture results. I billed CPT 99212 with diagnosis code B35.1. The EOB read $0 payment. Code 49 These are non-covered services because this is a routine exam or screening procedure done in conjunction with a routine exam. Any advice?
So, a patient presents to the office with a discolored toenail. The patient was evaluated for a fungal infection and wanted to have it treated before it exacerbated. Apparently a medically appropriate history and examination were performed as well as obtaining a nail specimen for a fungal culture to be performed. The various treatment options were discussed with the patient depending upon the results of the fungal culture. Based upon 1 self-limited or minor problem and a minimal risk of morbidity from diagnostic testing or treatment, E/M code 99212 was billed. The health insurance carrier is Novitas. I am assuming that this is a Medicare patient. The ICD-10-CM code that was billed was B35.1 . Novitas did not reimburse for the service based upon the premise that what was performed was a routine exam or screening procedure done in conjunction with a routine exam. What gives?
