Treatment Options For Toenail Fungus

The Best Treatment for Toenail Fungus, According to a Podiatrist

Toenail fungus affects nearly 10 percent of the worlds population and up to 32 million people in the United States alone. Historically, treatment of toenail fungus has been disappointing for both doctors and patients who suffer from it due to poor cure rates and high reoccurrence rates. Recently, the FDA approved some new topical treatments for toenail fungus. Here is a list of current treatments for toenail fungus and how they stack up.

Oral AntifungalsOral antifungals are still the gold standard of treatment for toenail fungus. Oral antifungals yield perhaps the best cure rates of any toenail fungus treatment. However, all of the oral antifungals have potential to cause liver damage and can interact with other medications. Many patients when informed of these potential side effects opt out of this treatment. Studies show that actual problems are rare. These medications depend upon a reservoir effect in which the medication becomes concentrated in the nail itself. The reservoir effect allows these oral medications to be taken for only a few months, but they continue to work until the nail grows out, usually up to a year or more. Current oral medications on the market include terbinafine and itraconazole . Cure rates with oral antifungals approach 40 percent which is the highest of all toenail fungus treatments.

When You Havefungal Nail

Fungal nail infection is a common condition. It’s not serious and not the end of the world, but the symptoms are annoying and can be even more so if the infection lingers or comes back. Thats why you shouldnt ignore it:

A fungal infection wont go on its own. Treat it, dont hide it.

Antifungal treatment doesn’t disappear just because symptoms do. Antifungal treatment needs several weeks to be completely effective.

Start treatment sooner rather than later to fight fungus more effectively.

If untreated, nail fungus can spread to other parts of your body and cause other types of skin fungus.

Make An Antifungal Foot Soak

Soaking your feet can also be a useful technique in reducing or eliminating toenail fungus. Many common household items can be used to make a healing antifungal foot bath. Mix one or more of the following with warm water and soak feet for 15 to 20 minutes. After foot bath, rinse feet with clean water and pat dry.

How Is Toenail Fungus Diagnosed

Nearly 3 million people in the United States are diagnosed with toenail fungus each year. Men are up to three times more likely to develop toenail fungus than women. You are also at a higher risk for a toenail fungus infection if you smoke or have diabetes. Also, individuals with compromised immune systems may experience toenail fungus that can turn into an infection.

When feet remain moist or damp, the fungus can grow, causing discoloration and thickening of toenails. Infection can cause discomfort and pain. The spread of fungus and infection can go from toe to toe, hand, fingernails, or skin on other parts of the body.

The toenail may become warped, cracked, or loosen itself away from the nail bed. Often the nail appears yellowish and may have white spots. Your doctor will examine your feet and potentially take a small sample of skin or nail to be sent off to a laboratory for testing. Your doctor may refer you to a dermatologist, podiatrist, or other healthcare professional if your condition is unusual or needs special care. Other foot conditions such as psoriasis can look similar to a fungal infection. Lab reports can indicate if the problem is fungal, helping the doctor prescribe the best medication for effective treatment.

Verywell Health / Peter Ardito

Toenail fungus is a common fungal condition that often starts after a rash on the foot spreads to the nails. When mild, it looks like white and yellow spots growing under the nail beds. If left untreated, the fungus can grow severely by hardening the nails and spreading to other toes.

Toenail Fungus Home Care

Sometimes itâs possible to treat your toenail fungus at home. You can try:

  • Over the counter antifungal creams or ointments. File or cut off any white markings on your nails. Soak your nails in water and dry off before you put on the medicated cream.
  • Special nail care. If you keep your nails trimmed, it can reduce the pressure and pain you feel. You can also thin your nail bed by filing it down. This can help antifungal creams get to the problem easier.

To Use Ciclopirox Topical Solution Follow These Steps:

  • Be sure that you have trimmed your nails properly before your first treatment.
  • Use the applicator brush attached to the bottle cap to apply ciclopirox topical solution evenly to all affected nails. Also apply the solution to the underside of the nail and the skin beneath it if you can reach these areas.
  • Wipe off the bottle cap and neck and replace the cap tightly on the bottle.
  • Let the solution dry for about 30 seconds before you put on socks or stockings.
  • When it is time for your next dose, apply ciclopirox topical solution over the medication that is already on your nails.
  • Once a week, remove all the ciclopirox from your nail with a cotton square or tissue soaked with rubbing alcohol. Then, remove as much of the damaged nail as possible using scissors, nail clippers, or nail files.

Amada Pure Toenail Fungus Treatment, Nail Fungus Treatment, Finger ...

In fact, there is are three topical medications on the market that has been shown in reputable studies to actually help kill the fungus AND make infected nails look better. It is important to note, however, that while they have been shown to help, none of them work particularly well. The best ones only work about 15% of the time.

The oldest one is the medication Ciclopirox. Ciclopirox Nail Lacquer is a topical nail solution that actually has been studied and found to be effective at clearing the nail in some patients. In addition, Ciclopirox is much cheaper than many OTC topicals. Ive seen in available for under $12 for a 3 month supply. You can read more detailed information here about Ciclopirox.

But here is the kicker even Ciclopirox only works about 8% of the time.

The reality is that no topical medication works well to clear nail fungus in most cases. None of them penetrate deep enough to make a difference in most people with the fungal infection.

We often use Ciclopirox to help prevent reinfection of the nail after treating the fungus with oral medication or laser, but do not consider any topical as an effective treatment on their own.

What Tests Will Be Done To Diagnose Toenail Fungus

Your healthcare provider will probably take a small sample from underneath your nail to further analyze it. Viewing the cells under a microscope can confirm a toenail fungus diagnosis. If the initial test is negative, a scraping can be sent to see if the fungus grows out in a culture. This also helps your healthcare provider identify the type of fungus.

    How Should I Take Terbinafine

    Treating Nail Fungus

Follow all directions on your prescription label. Do not take this medicine in larger or smaller amounts or for longer than recommended.

You may take the terbinafine tablet with or without food.

Terbinafine granules should be sprinkled into a spoonful of pudding or mashed potatoes . Swallow this mixture right away without chewing. Do not save the mixture for later use.

Terbinafine is usually taken for 6 weeks to treat scalp or fingernail infections, and for 12 weeks to treat a toenail infection.

Use this medicine for the full prescribed length of time. Your symptoms may improve before the infection is completely cleared. Skipping doses may also increase your risk of further infection that is resistant to antifungal medicine. Terbinafine will not treat a viral infection such as the flu or a common cold.

While using terbinafine, you may need frequent blood tests.

It may take several months for your nails to return to their normal appearance after your treatment with terbinafine.

Store at room temperature away from moisture, heat, and light. Keep the terbinafine oral granules in their sealed packet until you are ready to use.

    Topical Nail Fungus Medicines

Topical nail fungus medicines are available in several forms, including nail polishes and solutions applied by dropper. Prescription topicals include ciclopirox , efinaconazole , amorolfine and tavaborole . Youll have to you use them every day or every week for up to a year.

Using topical anti-fungal products alone isnt always effective, however, because nails are thick and difficult to penetrate. Unless your infection is mild, your dermatologist may suggest you combine a topical medicine with a prescription oral anti-fungal medication.

There are some exceptions. Topicals alone may be prescribed if you have a fungal nail infection caused by a mold, or if you have a less common form of nail fungus called white superficial onychomycosis, which affects only the surface of the nail and causes white spots that eventually become powdery.

How Can I Find Out Which Of The Treatments Will Work Best For Me

The most important thing to do when you have a fungal infection in the nail is to see a podiatrist who is experienced with all of the treatment options, and who utilizes a specialty lab for fungus testing.

The doctor, or preferably a podiatrist, should present you with all of the treatment options and the chances of success in your case. Then you will have the information necessary to decide on the treatment that works best with your lifestyle and comfort level.
